Teaching neuroimages: thalamic aphasia syndrome.

چکیده
An 83-year-old right-handed woman presented with sudden right-sided hemiparesis, somnolence, and loss of normal speech. Speech was nonfluent with semantic paraphasias and word-finding difficulties. Word repetition and comprehension were normal. MRI brain showed an area of restricted diffusion in the left thalamus consistent with acute infarction (figure 1). Speech fluency returned to normal after 2 days with occasional dysnomia and paraphasias. Left thalamic infarcts can result in aphasia that is characterized by lexical-semantic deficits and intact word repetition; fluency and comprehension are variably affected. Thalamic aphasia has been hypothesized to result from disconnection between cortical language centers and thalamic nuclei (figure 2). AUTHOR CONTRIBUTIONS Dr. Umair Afzal: acquisition of data, analysis and interpretation, drafting the manuscript. Dr. Muhammad U. Farooq: critical revision of the manuscript for important intellectual content and study supervision.

A 79-year-old man, admitted for left knee replacement, complained of right-sided vision loss on postoperative day 3. Examination revealed a right homonymous hemianopsia, fluent speech with semantic paraphasias, and impaired naming and comprehension, with intact repetition. MRI of the brain (figure) showed restricted diffusion affecting the left pulvinar nucleus, without signal abnormality in th...

A 58-year-old woman with a history of migraine presented with headache, aphasia, right homonymous hemianopsia, and right hemiparesis, followed by 2 seizures. On examination, she was noted to have a facial angioma (figure 1) that was present from childhood. MRI findings (figure 2, A–C) were consistent with leptomeningeal angiomatosis, making the diagnosis of Sturge-Weber syndrome. Sturge-Weber s...

A 66-year-old woman presented with 4 years of progressive speech difficulty. She had nonfluent speech with phonemic errors but intact single-word comprehension and object knowledge. Her grammar was impaired in both speech and writing, and she exhibited orofacial apraxia. A clinico-radiologic (see figure) diagnosis of nonfluent variant primary progressive aphasia was made. Nonfluent variant prim...
